Contratulations on the Birth of your Baby!

Before applying for a Birth Certificate, you must first register your baby's birth with Ontario's Office of the Registrar General. To link directly to the Birth Registration form through Service Ontario, click on the following link:

The fully electronic Newborn Registration Service took effect on Monday May 25, 2009.

  • Parents and authorized medical professionals will be able to submit birth registration documents electronically to ServiceOntario's Office of the Registrar General for any live birth occuring in Collingwood.
  • As of May 25, 2009, the Town of Collingwood will no longer be involved in the live birth registration process.
  • The parents of children born in locations where electronic registration is avilable will receive the information about the Newborn Registration Service from the hospital/birthing centre of the health professional that attended the birth.
  • Still births are not included in this electronic service and will continue to be reported to the municipality.
  • This new fully electronic Newborn Registration Service will help ServiceOntario meet it's goal in making it easier for Ontario families to gain access to the government information and services they need.

Once your baby has been registered and you have received acknowledgement from the Registrar General, you can then apply for the Birth Certificate.
