Final Report - Community Recreation and Culture Grants

All grant recipients are required to submit a follow-up report to show that grant funds have been used responsibly and according to the application. The report is due 60 days following the conclusion of the event or project for which you received a grant. Final reports must be received and approved in order for the organization to be considered for future grants. All of the questions are seen below on the first page. On the second page, you are asked to upload your final Project budget, which can be in any excel, PDF or Word format.

The outcomes should reflect those submitted in your application.
Please include numbers where possible. Those impacted may include participants, staff, volunteers, sponsors, funders or partners.
Attach the final budget for your Project. You may be asked for copies of receipts.
Files must be less than 2 MB.
