Application - Request for Letter (Welcome, Support and/or Congratulations)

Please review the criteria below and submit your request for letter via the APPLICATION FORM. Requests must be made at least three weeks in advance of  required dates to allow for preparation.

Required information:

Please fill out the APPLICATION FORM or send an email to officeofthemayor@collingwood.ca including the following information:

1) Requestor Information (Name, Organizations, Address, Phone & Email)
2) Date letter is required
3) To whom the letter is to be addressed
4) Purpose of the letter / wording of the letter
5) Method letter to be received (e-mail, mail, pick-up)

Approval, editing and content revisions may be required before the letter is finalized.
